A 18 year old female who underwent a primary rhinoplasty and chin implant. Had nasal breathing difficulty prior to surgery. She was mostly concerned with her profile view. Primary rhinoplasty included osteotomies, placement of speader grafts, and nostril rim grafts. A tongue and groove technique of her lower lateral tip cartilages was used in order to set back her tip onto her nasal septum. This technique provides for the shortening of her nose while also providing for long term tip stability. 15 month post-op shows refinement of her frontal view and profile alignment to include a lower dorsum. Further narrowing should become more apparent on longer follow-up. Attention should also be drawn to the subtle tip alignment changes with her upper lip, resulting in less tenting and greater elongation of the appearance of the upper lip. To further aide in profile alignment, a small chin implant was used to augment her chin projection.
