Photography and Computer Imaging

During your consultation with Dr. Bared, he takes high quality photographs of your nose. Dr. Bared believes that good quality photos from multiple angles are imperative to the analysis of the nose and adds to the physical examination. During your consultation, Dr. Bared will perform a series of photos using a very high resolution camera under standardized lighting. High quality photography is even more important in today’s era of cellular phone cameras and ‘selfies’ as one must appreciate that the small lens in these phone cameras result in what is known as ‘fish-eye’ distortion of the image. It is through the use of proper lighting and photography that image distortions can be essentially eliminated.

Dr. Bared utilizes computer imaging as a means of communication. Dr. Bared feels that computer imaging is a great tool to show patient’s the potential results of the surgery. He finds that it is a great way to understand the patients goals while also giving them a sense of what can be realistically achieved with a rhinoplasty procedure. Computer imaging is not a guarantee of the outcome as everyone’s healing and anatomy poses different variables but it is a great means for you and for Dr. Bared to understand the goals of the procedure. Dr. Anthony Bared, M.D., F.A.C.S.Dr. Bared’s utmost priority is to achieve natural-appearing results in rhinoplasty. He utilizes the most modern techniques in rhinoplasty, which have shown to have long lasting results. Rhinoplasty is considered to be the most challenging of all facial plastic surgery procedures. The inherent challenge of rhinoplasty lies in the anatomical differences in each individual. Dr. Bared tailors each procedure to the patient’s anatomy and skin thickness. He reshapes the nose using one’s own cartilage, avoiding the use of any artificial implants. He does not use artificial implants in the nose to avoid the long term risks associated with implants such as extrusion or infection.

I always wanted to fix my nose but at the same time I was scared of going with the whole process. But Dr. Bared made it so easy and smooth that I recommended it to anyone.

Typically, in primary cases, he uses cartilage from the septum which is the cartilage that divides both sides of the nose. Dr. Bared strategically places cartilage grafts to accomplish support for the nose. Cartilage grafts may be in the form of tip support grafts or grafts which support the bridge of the nose. The purpose of the cartilage grafts are to prevent the compromise of the long term shape and function of the nose. Inevitably after any rhinoplasty procedure, the skin of the nose heals and contracts over time. The contraction of the skin is expected and due to normal healing underneath the skin which transpires after surgery. If the underlying shape of the nose is not adequately supported, then too many variables are left to the healing process—the tip may eventually appear pinched or upturned, the middle part of the bridge may appear overly narrowed, the dorsum may take on an overly scooped appearance, and breathing problems may ensue. These are all telltale signs of a rhinoplasty which Dr. Bared attempts to avoid in his patients. Dr. Bared’s approach is to reshape and refine the patient’s existing anatomy in order to make it more harmonious and in balance with the patient’s overall facial features.

The Rhinoplasty (Nose Job) Procedure with Dr. Bared

Dr. Bared has been performing facial plastic surgery for years, and he has an excellent reputation for producing top quality results. While your personal procedure may vary slightly from others, it will follow a general set of steps:

Anesthesia: This promotes patient comfort and safety while the procedure is being performed. Dr. Bared performs rhinoplasty procedures under general anesthesia as he feels this is the safest form of anesthesia when having surgery, which involves the nasal airway.
Incision: Dr. Bared performs the majority of his procedures via the “open” approach. The cut will be made on the part of your nose between your nostrils and also hidden within the nostrils. Then the bones and cartilage can be easily seen in the anatomic location.
Sutures: When the procedure is complete, Dr. Bared will close the surgery area very carefully. Incisions will be hidden in the contours of the nose so that they won’t be noticeable. Dr. Bared uses extremely fine sutures to close the nose and takes a very careful and meticulous approach in the closure of the incision. After this incision heals, patients find that the scar is virtually undetectable and hidden under the nose.

Expected Recovery and Results from a Rhinoplasty Procedure with Dr. Bared

The care instructions for your surgery will be detailed so that you know what you need to do for recovery. There is surprisingly little pain associated with the surgery, but rather some discomfort, which patients describe as soreness for the first few days. Dr. Bared provides patients with prescription strength pain medication but most patients are able to take Extra Strength Tylenol within the first few days after surgery. It is very important to avoid taking over the counter anti-inflammatory medications before or after your surgery. You will need to avoid the use of aspirin or ibuprofen for two weeks before and two weeks after surgery. A splint goes over the nose which supports the healing of the nasal bones and helps with the swelling. In most cases, at your one week appointment, the cast and the sutures are all removed. Once your cast and sutures have been removed, Dr. Bared may place tape over the nose for a few days to help with the swelling. As with any surgery there are risks associated with rhinoplasty. Risks specific to rhinoplasty are but are not limited to bleeding, infection, septal perforation (an opening between both sides of the septum on the inside of the nose), and injury to the skin of the nose, scarring, irregularities, nostril asymmetry, nasal asymmetries, and nasal obstruction. A small subset of patient’s may undergo a revision surgery. Initially, it will take a few weeks for swelling to go down and everything to realign. Full exercise is permitted four weeks after surgery. If your bones are reset, you should avoid eyeglasses or sunglasses from resting on the bridge of your nose for six weeks after surgery.

The overall healing is a gradual process, where a degree of swelling in the tip area may persist for 12 months after surgery. The long term refinements made to the nose may not be apparent for this duration. Dr. Bared will explain to you what to expect from your recovery. His great amount of experience with rhinoplasty surgery will allow him to set proper expectations for your recovery. Depending on your skin type, some individuals can expect more swelling than others and a longer period of swelling.
